Important

TLC Care caregivers do not diagnose conditions, change prescriptions or replace doctors or nurses. Medication is administered only where permitted by the care plan, caregiver training and applicable requirements. Where a therapeutic or condition-specific diet is required, TLC Care works within guidance provided by the individual's doctor or qualified nutrition professional.

Care for the Person, Not Just the Condition

A diagnosis is never the whole story.

A person living with a long-term condition still wants to enjoy conversations, familiar meals, hobbies, family connections and the routines that make home feel like home.

That's why TLC Care looks beyond the condition. Our caregivers are matched according to the person's care requirements, personality, language and preferences wherever possible.

How It Works

A simple, personal approach to ongoing care.

  1. Tell Us About Your Loved One

Call, WhatsApp or complete our consultation form. Tell us about their condition, daily routine, challenges and the kind of support you are looking for.

  2. We Understand Their Needs

A Care Coordinator conducts an initial assessment and helps understand the level of assistance required.

  3. We Build a Personalised Care Plan

Together, we define the care schedule, responsibilities, routines and family communication requirements.

  4. Meet the Right Caregiver

We introduce a trained caregiver selected according to the care requirements, personality, language and preferences of the individual.

  5. Care Begins

Your loved one receives consistent support in the comfort of home while the family stays connected.

  6. We Adapt as Needs Change

Chronic conditions can change over time. Your Care Coordinator can review the care arrangement and help adjust the plan when required.

When Should You Consider Chronic Illness Care?

You don't have to wait for a crisis before asking for help.

You may want to consider additional home support if your loved one shows any of the following signs.

  • A Care Coordinator reviewing a care plan with a family as their loved one's needs change
  • Struggles to manage everyday routines independently
  • Frequently forgets medications or appointments
  • Has difficulty preparing meals or maintaining hydration
  • Has reduced mobility or increased risk of falls
  • Needs assistance with bathing, dressing or toileting
  • Spends increasing amounts of time alone
  • Has difficulty attending medical appointments
  • Needs someone to observe and report changes in everyday wellbeing
  • Has recently returned home after hospitalisation
  • Is becoming increasingly dependent on family members for routine activities

Ask us on WhatsApp
What chronic conditions can TLC Care support?

TLC Care can support people living with many long-term conditions, including diabetes, hypertension, cardiovascular conditions, COPD, Parkinson's disease, arthritis, stroke-related limitations and other chronic conditions requiring assistance with everyday living. The appropriate care plan depends on the individual's specific needs.

Does TLC Care provide medical treatment?

TLC Care's home care service is primarily focused on personal care, daily living assistance, companionship, routine support and care coordination. It does not replace a doctor, hospital or qualified medical professional. Clinical services, where available, should be separately assessed and arranged with appropriate professionals.

Can caregivers give medicines?

Caregivers can provide medication reminders and, where permitted, support medication routines according to their training, the care plan and applicable requirements. They do not independently change prescriptions or make medical decisions.

Can I arrange care for my parents from another country?

Yes. TLC Care is designed to support families who may not live close to their loved ones. Initial discussions, assessments and coordination can be handled remotely, with regular updates helping families stay informed.

Can the caregiver stay overnight?

Depending on the assessment, caregiver availability and the family's requirements, TLC Care can arrange extended or around-the-clock support where available.

What happens if my loved one's condition changes?

Your Care Coordinator can review the care arrangement and help determine whether the existing plan should be modified or whether additional professional support is required.

Is chronic illness care only for elderly people?

No. Chronic illness can affect people of different ages. TLC Care can assess care requirements for individuals who need ongoing assistance with everyday activities because of a long-term health condition.

Can TLC Care help with hospital appointments?

Depending on the care plan, caregivers may be able to accompany or assist your loved one during appointments and help the family stay informed about practical requirements.

Better Care for Today. More Confidence for Tomorrow.

Living with chronic illness is a journey. With the right support, your loved one can continue enjoying the familiarity, comfort and dignity of home — while your family gains the reassurance of knowing someone dependable is there. Let's create a care plan that works for your family.
